c66a037dca Bug#17474166 - EXECUTING STATEMENT LIKE 'SHOW ENGINE INNODB'
AND 'KILL SESSION' LEAD TO CRASH               

Analysis:
--------
This situation occurs when the connection executes query 
"show engine innodb status" and this connection is killed by
executing statement "kill <con>" by another connection.

In function "innodb_show_status", function "stat_print"
is called to print the status but return value of function
is not checked.  After killing connection, if write to 
connection fails then error is returned and same is set
in Diagnostic area. Since FALSE is returned from
"innodb_show_status" now, assert to check no error
is set in function "set_eof_status" (called from
my_eof) is failing. 

Fix:

Changed code to check return value of function "stat_print"
in "innodb_show_status".

fc2c669297 Fix for bug#14188793 - "DEADLOCK CAUSED BY ALTER TABLE DOEN'T CLEAR
STATUS OF ROLLBACKED TRANSACTION" and bug #17054007 - "TRANSACTION
IS NOT FULLY ROLLED BACK IN CASE OF INNODB DEADLOCK".

The problem in the first bug report was that although deadlock involving
metadata locks was reported using the same error code and message as InnoDB
deadlock it didn't rollback transaction like the latter. This caused
confusion to users as in some cases after ER_LOCK_DEADLOCK transaction
could have been restarted immediately and in some cases rollback was
required.

The problem in the second bug report was that although InnoDB deadlock
caused transaction rollback in all storage engines it didn't cause release
of metadata locks. So concurrent DDL on the tables used in transaction was
blocked until implicit or explicit COMMIT or ROLLBACK was issued in the
connection which got InnoDB deadlock.

The former issue has stemmed from the fact that when support for detection
and reporting metadata locks deadlocks was added we erroneously assumed
that InnoDB doesn't rollback transaction on deadlock but only last statement
(while this is what happens on InnoDB lock timeout actually) and so didn't
implement rollback of transactions on MDL deadlocks.

The latter issue was caused by the fact that rollback of transaction due
to deadlock is carried out by setting THD::transaction_rollback_request
flag at the point where deadlock is detected and performing rollback
inside of trans_rollback_stmt() call when this flag is set. And
trans_rollback_stmt() is not aware of MDL locks, so no MDL locks are
released.

This patch solves these two problems in the following way:

- In case when MDL deadlock is detect transaction rollback is requested
  by setting THD::transaction_rollback_request flag.

- Code performing rollback of transaction if THD::transaction_rollback_request
  is moved out from trans_rollback_stmt(). Now we handle rollback request
  on the same level as we call trans_rollback_stmt() and release statement/
  transaction MDL locks.

b7f9c89423 MDEV-4702 - Reduce usage of LOCK_open
Following variables do not require LOCK_open protection anymore:
- table_def_cache (renamed to tdc_hash) is protected by rw-lock
  LOCK_tdc_hash;
- table_def_shutdown_in_progress doesn't need LOCK_open protection;
- last_table_id use atomics;
- TABLE_SHARE::ref_count (renamed to TABLE_SHARE::tdc.ref_count)
  is protected by TABLE_SHARE::tdc.LOCK_table_share;
- TABLE_SHARE::next, ::prev (renamed to tdc.next and tdc.prev),
  oldest_unused_share, end_of_unused_share are protected by
  LOCK_unused_shares;
- TABLE_SHARE::m_flush_tickets (renamed to tdc.m_flush_tickets)
  is protected by TABLE_SHARE::tdc.LOCK_table_share;
- refresh_version (renamed to tdc_version) use atomics.

  Bug#14741537 - MYSQL 5.6, GTID AND PERFORMANCE_SCHEMA
  
  Before this fix, statements using performance_schema tables:
  - were marked as unsafe for replication,
  - did cause warnings during execution,
  - were written to the binlog, either in STATEMENT or ROW format.
  
  When using replication with the new GTID feature,
  unsafe warnings are elevated to errors,
  which prevents to use both the performance_schema and GTID together.
  
  The root cause of the problem is not related to raising warnings/errors
  in some special cases, but deeper: statements involving the performance
  schema should not even be written to the binary log in the first place,
  because the content of the performance schema tables is 'local' to a server
  instance, and may differ greatly between nodes in a replication
  topology.
  
  In particular, the DBA should be able to configure (INSERT, UPDATE, DELETE)
  or flush (TRUNCATE) performance schema tables on one node,
  without affecting other nodes.
  
  This fix introduces the concept of a 'non-replicated' or 'local' table,
  and adjusts the replication logic to ignore tables that are not replicated
  when deciding if or how to log a statement to the binlog.
  
  Note that while this issue was detected using the performance_schema,
  other tables are also affected by the same problem.
  
  This fix define as 'local' the following tables, which are then never
  replicated:
  - performance_schema.*
  - mysql.general_log
  - mysql.slow_log
  - mysql.slave_relay_log_info
  - mysql.slave_master_info
  - mysql.slave_worker_info
  
  Existing behavior for information_schema.* is unchanged by this fix,
  to limit the scope of changes.
  
  Coding wise, this fix implements the following changes:
  
  1)
  
  Performance schema tables are not using any replication flags,
  since performance schema tables are not replicated.
  
  2)
  
  In open_table_from_share(),
  tables with no replication capabilities (performance_schema.*),
  tables with TABLE_CATEGORY_LOG (logs)
  and tables with TABLE_CATEGORY_RPL_INFO (replication)
  are marked as non replicated, with TABLE::no_replicate
  
  3)
  
  A new THD member, THD::m_binlog_filter_state,
  indicate if the current statement is written to the binlog
  (normal cases for most statements), or is to be discarded
  (because the statements affects non replicated tables).
  
  4)
  
  In THD::decide_logging_format(), the replication logic
  is changed to take into account non replicated tables.
  
  Statements that affect only non replicated tables are
  executed normally (no warning or errors), but not written
  to the binlog.
  
  Statements that affect (i.e., write to) a replicated table
  while also using (i.e., reading from or writing to) a non replicated table
  are executed normally in MIXED and ROW binlog format,
  and cause a new error in STATEMENT binlog format.
  
  THD::decide_logging_format() uses THD::m_binlog_filter_state
  to indicate if a statement is to be ignored, when writing to
  the binlog.
  
  5)
  
  In THD::binlog_query(), statements marked as ignored
  are not written to the binary log.
  
  6)
  
  For row based replication, the existing test for 'table->no_replicate',
  has been moved from binlog_log_row() to check_table_binlog_row_based().

50dda403b7 Fix test errors like:
-Note 1031 Table storage engine for 't1' doesn't have this option
+Note 1031 Table storage engine for 'InnoDB' doesn't have this option

They were caused by a change in MariaDB which changed ER_ILLEGAL_HA message 
text to be like:
"Storage engine InnoDB of the table `test`.`t1` doesn't have this option"

Some the error calls were changed to pass new parameters, but some were left
to be old. Also the error text in errmsg-ut8.txt was not changed.

7e5dc4f074 MDEV-4506: Parallel replication. Intermediate commit.
Implement facility for the commit in one thread to wait for the commit of
another to complete first. The wait is done in a way that does not hinder
that a waiter and a waitee can group commit together with a single fsync()
in both binlog and InnoDB. The wait is done efficiently with respect to
locking.

The patch was originally made to support TaoBao parallel replication with
in-order commit; now it will be adapted to also be used for parallel
replication of group-committed transactions.

A waiter THD registers itself with a prior waitee THD. The waiter will then
complete its commit at the earliest in the same group commit of the waitee
(when using binlog). The wait can also be done explicitly by the waitee.

e96182824d Bug#16274455: CAN NOT ACESS PARTITIONED TABLES WHEN
DOWNGRADED FROM 5.6.11 TO 5.6.10

Problem was new syntax not accepted by previous version.

Fixed by adding version comment of /*!50531 around the
new syntax.

Like this in the .frm file:
'PARTITION BY KEY /*!50611 ALGORITHM = 2 */ () PARTITIONS 3'
and also changing the output from SHOW CREATE TABLE to:
CREATE TABLE t1 (a INT)
/*!50100 PARTITION BY KEY */ /*!50611 ALGORITHM = 1 */ /*!50100 ()
PARTITIONS 3 */

It will always add the ALGORITHM into the .frm for KEY [sub]partitioned
tables, but for SHOW CREATE TABLE it will only add it in case it is the non
default ALGORITHM = 1.

Also notice that for 5.5, it will say /*!50531 instead of /*!50611, which
will make upgrade from 5.5 > 5.5.31 to 5.6 < 5.6.11 fail!
If one downgrades an fixed version to the same major version (5.5 or 5.6) the
bug 14521864 will be visible again, but unless the .frm is updated, it will
work again when upgrading again.

Also fixed so that the .frm does not get updated version
if a single partition check passes.

d92a7cb76a Bug#14521864: MYSQL 5.1 TO 5.5 BUGS PARTITIONING
Due to an internal change in the server code in between 5.1 and 5.5
(wl#2649) the hash function used in KEY partitioning changed
for numeric and date/time columns (from binary hash calculation
to character based hash calculation).

Also enum/set changed from latin1 ci based hash calculation to
binary hash between 5.1 and 5.5. (bug#11759782).

These changes makes KEY [sub]partitioned tables on any of
the affected column types incompatible with 5.5 and above,
since the calculation of partition id differs.

Also since InnoDB asserts that a deleted row was previously
read (positioned), the server asserts on delete of a row that
is in the wrong partition.

The solution for this situation is:

1) The partitioning engine will check that delete/update will go to the
partition the row was read from and give an error otherwise, consisting
of the rows partitioning fields. This will avoid asserts in InnoDB and
also alert the user that there is a misplaced row. A detailed error
message will be given, including an entry to the error log consisting
of both table name, partition and row content (PK if exists, otherwise
all partitioning columns).


2) A new optional syntax for KEY () partitioning in 5.5 is allowed:
[SUB]PARTITION BY KEY [ALGORITHM = N] (list_of_cols)
Where N = 1 uses the same hashing as 5.1 (Numeric/date/time fields uses
binary hashing, ENUM/SET uses charset hashing) N = 2 uses the same
hashing as 5.5 (Numeric/date/time fields uses charset hashing,
ENUM/SET uses binary hashing). If not set on CREATE/ALTER it will
default to 2.

This new syntax should probably be ignored by NDB.


3) Since there is a demand for avoiding scanning through the full
table, during upgrade the ALTER TABLE t PARTITION BY ... command is
considered a no-op (only .frm change) if everything except ALGORITHM
is the same and ALGORITHM was not set before, which allows manually
upgrading such table by something like:
ALTER TABLE t PARTITION BY KEY ALGORITHM = 1 () or
ALTER TABLE t PARTITION BY KEY ALGORITHM = 2 ()


4) Enhanced partitioning with CHECK/REPAIR to also check for/repair
misplaced rows. (Also works for ALTER TABLE t CHECK/REPAIR PARTITION)

CHECK FOR UPGRADE:
If the .frm version is < 5.5.3
and uses KEY [sub]partitioning
and an affected column type
then it will fail with an message:
KEY () partitioning changed, please run:
ALTER TABLE `test`.`t1`  PARTITION BY KEY ALGORITHM = 1 (a)
PARTITIONS 12
(i.e. current partitioning clause, with the addition of
ALGORITHM = 1)

CHECK without FOR UPGRADE:
if MEDIUM (default) or EXTENDED options are given:
Scan all rows and verify that it is in the correct partition.
Fail for the first misplaced row.

REPAIR:
if default or EXTENDED (i.e. not QUICK/USE_FRM):
Scan all rows and every misplaced row is moved into its correct
partitions.


5) Updated mysqlcheck (called by mysql_upgrade) to handle the
new output from CHECK FOR UPGRADE, to run the ALTER statement
instead of running REPAIR.

This will allow mysql_upgrade (or CHECK TABLE t FOR UPGRADE) to upgrade
a KEY [sub]partitioned table that has any affected field type
and a .frm version < 5.5.3 to ALGORITHM = 1 without rebuild.


Also notice that if the .frm has a version of >= 5.5.3 and ALGORITHM
is not set, it is not possible to know if it consists of rows from
5.1 or 5.5! In these cases I suggest that the user does:
(optional)
LOCK TABLE t WRITE;
SHOW CREATE TABLE t;
(verify that it has no ALGORITHM = N, and to be safe, I would suggest
backing up the .frm file, to be used if one need to change to another
ALGORITHM = N, without needing to rebuild/repair)
ALTER TABLE t <old partitioning clause, but with ALGORITHM = N>;
which should set the ALGORITHM to N (if the table has rows from
5.1 I would suggest N = 1, otherwise N = 2)
CHECK TABLE t;
(here one could use the backed up .frm instead and change to a new N
and run CHECK again and see if it passes)
and if there are misplaced rows:
REPAIR TABLE t;
(optional)
UNLOCK TABLES;

a260b15554 MDEV-4011 Added per thread memory counting and usage
Base code and idea from a patch from by plinux at Taobao.

The idea is that we mark all memory that are thread specific with MY_THREAD_SPECIFIC.
Memory counting is done per thread in the my_malloc_size_cb_func callback function from my_malloc().
There are plenty of new asserts to ensure that for a debug server the counting is correct.

Information_schema.processlist gets two new columns: MEMORY_USED and EXAMINED_ROWS.
- The later is there mainly to show how query is progressing.

The following changes in interfaces was needed to get this to work:
- init_alloc_root() amd init_sql_alloc() has extra option so that one can mark memory with MY_THREAD_SPECIFIC
- One now have to use alloc_root_set_min_malloc() to set min memory to be allocated by alloc_root()
- my_init_dynamic_array()  has extra option so that one can mark memory with MY_THREAD_SPECIFIC
- my_net_init() has extra option so that one can mark memory with MY_THREAD_SPECIFIC
- Added flag for hash_init() so that one can mark hash table to be thread specific.

Analysis:

When the server is out of memory, an error is raised
to indicate the same. Handling the error requires
more memory to be allocated which fails, hence the
error handling loops in a recursion and causes the
server to crash.

Fix:
---
a) Prevents pushing the 'out of memory' error condition
to the diagnostic area as it requires memory allocation.
GET DIAGNOSTICS, SHOW WARNINGS and SHOW ERRORS statements
will not show information about this error. However the
'out of memory' error is returned to the client.
b) It sets the ME_FATALERROR flag when 'out of memory' errors
are reported (for places where the flag is not already set).
This flag prevents activation of SP error handlers which also
require memory allocation and therefore are likely to fail.

47f5632d60 MDEV-3837 Assertion `table->read_set == &table->def_read_set' failed on updating a performance_schema table
This was failing not only for P_S, but for any engine that had
HA_PRIMARY_KEY_REQUIRED_FOR_DELETE flag set (in the tree - only P_S and federated).
Because of this flag, read_set and write_set were (possibly) changed
on update. But later the code modified these bitmaps and restored them to the default
state, losing HA_PRIMARY_KEY_REQUIRED_FOR_DELETE related changes.

  don't change the *write* set only because all columns has to be *read*

378a7d1ef5 Bug #14036214 MYSQLD CRASHES WHEN EXECUTING UPDATE IN TRX WITH
CONSISTENT SNAPSHOT OPTION

A transaction is started with a consistent snapshot.  After 
the transaction is started new indexes are added to the 
table.  Now when we issue an update statement, the optimizer
chooses an index.  When the index scan is being initialized
via ha_innobase::change_active_index(), InnoDB reports 
the error code HA_ERR_TABLE_DEF_CHANGED, with message 
stating that "insufficient history for index".

This error message is propagated up to the SQL layer.  But
the my_error() api is never called.  The statement level
diagnostics area is not updated with the correct error 
status (it remains in Diagnostics_area::DA_EMPTY).  

Hence the following check in the Protocol::end_statement()
fails.

  MariaDB initializes the "manager" resources from the "manager" thread, and starts this thread only when --flush-time is not 0. MySQL 5.6 initializes "manager" resources unconditionally on server startup.

ae5bc05988 Fix for MDEV-533: Confusing error code when doing auto-increment insert for out-of-range values
create table t1 (a smallint primary key auto_increment);
insert into t1 values(32767);
insert into t1 values(NULL);
ERROR 1062 (23000): Duplicate entry '32767' for key 'PRIMARY

Now on always gets error HA_ERR_AUTOINC_RANGE=167 "Out of range value for column", independent of
store engine, SQL Mode or number of inserted rows. This is an unique error that is easier to test for in replication.

Another bug fix is that we now get an error when trying to insert a too big auto-generated value, even in non-strict mode.
Before one get insted the max column value inserted.
This patch also fixes some issues with inserting negative numbers in an auto-increment column.
Fixed the ER_DUP_ENTRY and HA_ERR_AUTOINC_ERANGE are compared the same between master and slave.
This ensures that replication works between an old server to a new slave for auto-increment overflow errors.
Added SQLSTATE errors for handler errors

Smaller bug fixes:
* Added warnings for duplicate key errors when using INSERT IGNORE
* Fixed bug when using --skip-log-bin followed by --log-bin, which did set log-bin to "0"
* Allow one to see how cmake is called by using --just-print --just-configure

288eeb3a31 MDEV-232: Remove one fsync() from commit phase.
Introduce a new storage engine API method commit_checkpoint_request().
This is used to replace the fsync() at the end of every storage engine
commit with a single fsync() when a binlog is rotated.

Binlog rotation is now done during group commit instead of being
delayed until unlog(), removing some server stall and avoiding an
expensive lock/unlock of LOCK_log inside unlog().

357a008ad3 Bug #11765218 58157: INNODB LOCKS AN UNMATCHED ROW EVEN THOUGH USING
RBR AND RC

Description: When scanning and locking rows with < or <=, InnoDB locks
the next row even though row based binary logging and read committed
is used.

Solution: In the handler, when the row is identified to fall outside
of the range (as specified in the query predicates), then request the
storage engine to unlock the row (if possible). This is done in
handler::read_range_first() and handler::read_range_next().

0697ee265f MDEV-181: XID crash recovery across binlog boundaries
Keep track of how many pending XIDs (transactions that are prepared in
storage engine and written into binlog, but not yet durably committed
on disk in the engine) there are in each binlog.

When the count of one binlog drops to zero, write a new binlog checkpoint
event, telling which is the oldest binlog with pending XIDs.

When doing XA recovery after a crash, check the last binlog checkpoint
event, and scan all binlog files from that point onwards for XIDs that
must be committed if found in prepared state inside engine.

Remove the code in binlog rotation that waits for all prepared XIDs to
be committed before writing a new binlog file (this is no longer necessary
when recovery can scan multiple binlog files).

a5731b27b7 MDEV-342: Do not mark old binlog file as cleanly closed during rotate until
the new file is fully synced to disk and binlog index. This fixes a window
where a crash would leave next server restart unable to detect that a crash
occured, causing recovery to fail.
