Fix for Bug#43138: DROP DATABASE failure does not clean up message list.
The problem was that the high-level function mysql_rm_db() invoked
low-level mysql_rm_table_part2(), which reported low-level error
(Unknown table) if SE refused to delete a table. Also when
mysql_rm_table_part2() reported an error, it didn't add corresponding
warning into the list (because it is used from other places where such
behaviour is required).
The fix is to
1. Remove no_warnings_for_error usage from sql_table.cc
2. Improve internal error handler support in THD, so that
a stack of error handlers is allowed.
3. Create an internal error handler (Drop_table_error_handler)
to silence useless warnings.
4. Use the handler in DROP DATABASE and DROP TABLE statements.

This patch implements:
WL#4264 "Backup: Stabilize Service Interface" -- all the
server prerequisites except si_objects.{h,cc} themselves (they can
be just copied over, when needed).
WL#4435: Support OUT-parameters in prepared statements.
(and all issues in the initial patches for these two
tasks, that were discovered in pushbuild and during testing).
Bug#39519: mysql_stmt_close() should flush all data
associated with the statement.
After execution of a prepared statement, send OUT parameters of the invoked
stored procedure, if any, to the client.
When using the binary protocol, send the parameters in an additional result
set over the wire. When using the text protocol, assign out parameters to
the user variables from the CALL(@var1, @var2, ...) specification.
The following refactoring has been made:
- Protocol::send_fields() was renamed to Protocol::send_result_set_metadata();
- A new Protocol::send_result_set_row() was introduced to incapsulate
common functionality for sending row data.
- Signature of Protocol::prepare_for_send() was changed: this operation
does not need a list of items, the number of items is fully sufficient.
The following backward incompatible changes have been made:
- CLIENT_MULTI_RESULTS is now enabled by default in the client;
- CLIENT_PS_MULTI_RESUTLS is now enabled by default in the client.
include/mysql.h:
Add a new flag to MYSQL_METHODS::flush_use_result
function pointer. This flag determines if all results
should be flushed or only the first one:
- if flush_all_results is TRUE, then cli_flush_use_result()
will read/flush all pending results. I.e. it will read
all packets while server status attribute indicates that
there are more results. This is a new semantic, required
to fix the bug.
- if flush_all_results is FALSE, the old sematic
is preserved -- i.e. cli_flush_use_result() reads data
until first EOF-packet.

Implemented the server infrastructure for the fix:
1. Added a function LEX_STRING *thd_query_string(THD) to return
a LEX_STRING structure instead of char *.
This is the function that must be called in innodb instead of
thd_query()
2. Did some encapsulation in THD : aggregated thd_query and
thd_query_length into a LEX_STRING and made accessor and mutator
methods for easy code updating.
3. Updated the server code to use the new methods where applicable.
Post-push fix.
Problem: After the original bugfix, if a statement is unsafe,
binlog_format=mixed, and engine is statement-only, a warning was
generated and the statement executed. However, it is a fundamental
principle of binlogging that binlog_format=mixed should guarantee
correct logging, no compromise. So correct behavior is to generate
an error and don't execute the statement.
Fix: Generate error instead of warning.
Since issue_unsafe_warnings can only generate one error message,
this allows us to simplify the code a bit too:

Server was unable to find existing or explicitly created supporting
index for foreign key if corresponding statement clause used field
names in case different than one used in key specification and created
yet another supporting index.
In cases when name of constraint (and thus name of generated index)
was the same as name of existing/explicitly created index this led
to duplicate key name error.
The problem was that unlike all other code Key_part_spec::operator==()
compared field names in case sensitive fashion. As result routines
responsible for getting rid of redundant generated supporting indexes
for foreign key were not working properly for versions of field names
using different cases.

The idea of this patch is to change type of Key_part_spec::field_name and
Key::name to LEX_STRING in order to avoid extra strlen() calls during
semantic analysis and statement execution, particularly, in code to be
implemented on the 2nd milestone of WL#148.
Note that since we are not using LEX_STRING everywhere yet (e.g. in
Create_field and KEY) and we want to limit scope of our changes we
have to do strlen() in places where we create Key and Key_part_spec
instances from objects using plain (char*) for strings. These calls
will go away during the process of further (char*) -> LEX_STRING
refactoring.
We have introduced these changes in 6.0 and backported them to 5.5
tree to make people aware of these changes as early as possible and
to simplify merges with mysql-fk and mysql-6.1-fk trees.
No test case is needed since this patch does not introduce any
user visible changes.

1) Renamed have_community_features server system variable to

ENABLE_COMMUNITY_FEATURES macro.

Only --enable-profiling is now needed to enable profiling.
It was the only existing "community feature", so there was
no need for both configure options.
Using --enable-community-features will give a warning message

WL#4828 Augment DBUG_ENTER/DBUG_EXIT to crash MySQL in different functions

The assessment of the replication code in the presence of faults is extremely
import to increase reliability. In particular, one needs to know if servers
will either correctly recovery or print out appropriate error messages thus
avoiding unexpected problems in a production environment.
In order to accomplish this, the current patch refactories the debug macros
already provided in the source code and introduces three new macros that
allows to inject faults, specifically crashes, while entering or exiting a
function or method. For instance, to crash a server while returning from
the init_slave function (see module sql/slave.cc), one needs to do what
follows:
1 - Modify the source replacing DBUG_RETURN by DBUG_CRASH_RETURN;
DBUG_CRASH_RETURN(0);
2 - Use the debug variable to activate dbug instructions:
SET SESSION debug="+d,init_slave_crash_return";
The new macros are briefly described below:
DBUG_CRASH_ENTER (function) is equivalent to DBUG_ENTER which registers the
beginning of a function but in addition to it allows for crashing the server
while entering the function if the appropriate dbug instruction is activate.
In this case, the dbug instruction should be "+d,function_crash_enter".
DBUG_CRASH_RETURN (value) is equivalent to DBUG_RETURN which notifies the
end of a function but in addition to it allows for crashing the server
while returning from the function if the appropriate dbug instruction is
activate. In this case, the dbug instruction should be
"+d,function_crash_return". Note that "function" should be the same string
used by either the DBUG_ENTER or DBUG_CRASH_ENTER.

dynamic instrumentation of the code.
BUG#45747 DBUG_CRASH_* is not setting the strict option
---------
When combining DBUG_CRASH_* with "--debug=d:t:i:A,file" the server crashes
due to a call to the abort function in the DBUG_CRASH_* macro althought the
appropriate keyword has not been set.

along with transaction isolation level READ-COMMITTED (or weaker),
even if the statement in question is filtered out according to the
binlog-do-db rules set. In this case, an error should not be printed.
This patch addresses this issue by extending the existing check in
external_lock to take into account the filter rules before deciding to
print an error. Furthermore, it also changes decide_logging_format to
take into consideration whether the statement is filtered out from
binlog before decision is made.

This is a post-push fix addressing review requests and

Problem 1: The sub-statement where an unsafe warning was detected was
printed as part of the warning. This was ok for statements that
were unsafe due to, e.g., calls to UUID(), but did not make
sense for statements that were unsafe because there was more than
one autoincrement column (unsafeness in this case comes from the
combination of several sub-statements).
Fix 1: Instead of printing the sub-statement, print an explanation
of why the statement is unsafe.
Problem 2:
When a recursive construct (i.e., stored proceure, stored
function, trigger, view, prepared statement) contained several
sub-statements, and at least one of them was unsafe, there would be
one unsafeness warning per sub-statement - even for safe
sub-statements.
Fix 2:
Ensure that each type of warning is printed at most once, by
remembering throughout the execution of the statement which types
of warnings have been printed.

General overview:
The logic for switching to row format when binlog_format=MIXED had
numerous flaws. The underlying problem was the lack of a consistent
architecture.
General purpose of this changeset:
This changeset introduces an architecture for switching to row format
when binlog_format=MIXED. It enforces the architecture where it has
to. It leaves some bugs to be fixed later. It adds extensive tests to
verify that unsafe statements work as expected and that appropriate
errors are produced by problems with the selection of binlog format.
It was not practical to split this into smaller pieces of work.
Problem 1:
To determine the logging mode, the code has to take several parameters
into account (namely: (1) the value of binlog_format; (2) the
capabilities of the engines; (3) the type of the current statement:
normal, unsafe, or row injection). These parameters may conflict in
several ways, namely:
- binlog_format=STATEMENT for a row injection
- binlog_format=STATEMENT for an unsafe statement
- binlog_format=STATEMENT for an engine only supporting row logging
- binlog_format=ROW for an engine only supporting statement logging
- statement is unsafe and engine does not support row logging
- row injection in a table that does not support statement logging
- statement modifies one table that does not support row logging and
one that does not support statement logging
Several of these conflicts were not detected, or were detected with
an inappropriate error message. The problem of BUG#39934 was that no
appropriate error message was written for the case when an engine
only supporting row logging executed a row injection with
binlog_format=ROW. However, all above cases must be handled.
Fix 1:
Introduce new error codes (sql/share/errmsg.txt). Ensure that all
conditions are detected and handled in decide_logging_format()
Problem 2:
The binlog format shall be determined once per statement, in
decide_logging_format(). It shall not be changed before or after that.
Before decide_logging_format() is called, all information necessary to
determine the logging format must be available. This principle ensures
that all unsafe statements are handled in a consistent way.
However, this principle is not followed:
thd->set_current_stmt_binlog_row_based_if_mixed() is called in several
places, including from code executing UPDATE..LIMIT,
INSERT..SELECT..LIMIT, DELETE..LIMIT, INSERT DELAYED, and
SET @@binlog_format. After Problem 1 was fixed, that caused
inconsistencies where these unsafe statements would not print the
appropriate warnings or errors for some of the conflicts.
Fix 2:
Remove calls to THD::set_current_stmt_binlog_row_based_if_mixed() from
code executed after decide_logging_format(). Compensate by calling the
set_current_stmt_unsafe() at parse time. This way, all unsafe statements
are detected by decide_logging_format().
Problem 3:
INSERT DELAYED is not unsafe: it is logged in statement format even if
binlog_format=MIXED, and no warning is printed even if
binlog_format=STATEMENT. This is BUG#45825.
Fix 3:
Made INSERT DELAYED set itself to unsafe at parse time. This allows
decide_logging_format() to detect that a warning should be printed or
the binlog_format changed.
Problem 4:
LIMIT clause were not marked as unsafe when executed inside stored
functions/triggers/views/prepared statements. This is
BUG#45785.
Fix 4:
Make statements containing the LIMIT clause marked as unsafe at
parse time, instead of at execution time. This allows propagating
unsafe-ness to the view.

This call can be safely removed, because:
- current_stmt_binlog_row_based is initialized for the ndb_binlog
thread's THD object when the THD object is created. So we're
not going to read uninitialized memory.
- The behavior of ndb_binlog thread does not use the state of the
current_stmt_binlog_row_based. It is conceivable that the
ndb_binlog thread would rely on the current_stmt_binlog_format
in two situations:
(1) when it calls mysql_parse;
(2) when it calls THD::binlog_query.
In case (1), it always clears THD::options&OPTION_BIN_LOG (because
run_query() in ha_ndbcluster_binlog.cc is only called with
disable_binlogging = TRUE).
In case (2), it always uses qtype=STMT_QUERY_TYPE.

case has been removed, and the behavior of

calls it any more. The special case did not serve any purpose,
since the ndb binlog thread did not take the
current_stmt_binlog_row_based flag into account anyways.

Logging to slow log can produce an undetermined value for
Rows_examined in special cases. In debug mode this manifests
itself as any of the various marker values used to mark
uninitialized memory on various platforms.
If logging happens on a THD object that hasn't performed any
row reads (on this or any previous connections), the
THD::examined_row_count may be uninitialized. This patch adds
initialization for this attribute.
No automated test cases are added, as for this to be
meaningful, we need to ensure that we're using a THD
fulfilling the above conditions. This is hard to do in the
mysql-test-run framework. The patch has been verified
manually, however, by restarting mysqld and running the test
included with the bug report.

MySQL crashes if a user without proper privileges attempts to create a procedure.
The crash happens because more than one error state is pushed onto the Diagnostic
area. In this particular case the user is denied to implicitly create a new user
account with the implicitly granted privileges ALTER- and EXECUTE ROUTINE.
The new account is needed if the original user account contained a host mask.
A user account with a host mask is a distinct user account in this context.
An alternative would be to first get the most permissive user account which
include the current user connection and then assign privileges to that
account. This behavior change is considered out of scope for this bug patch.
The implicit assignment of privileges when a user creates a stored routine is a
considered to be a feature for user convenience and as such it is not
a critical operation. Any failure to complete this operation is thus considered
non-fatal (an error becomes a warning).
The patch back ports a stack implementation of the internal error handler interface.
This enables the use of multiple error handlers so that it is possible to intercept
and cancel errors thrown by lower layers. This is needed as a error handler already
is used in the call stack emitting the errors which needs to be converted.

Problem:
"In general, statements executed within a stored procedure
are written to the binary log using the same rules that
would apply were the statements to be executed in standalone
fashion. Some special care is taken when logging procedure
statements because statement execution within procedures
is not quite the same as in non-procedure context".
For example, each reference to a local variable in SP's
statements is replaced by NAME_CONST(var_name, var_value).
Queries like
"CREATE TABLE ... SELECT FUNC(local_var ..."
are logged as
"CREATE TABLE ... SELECT FUNC(NAME_CONST("local_var", var_value) ..."
that leads to differrent field names and
might result in "Incorrect column name" if var_value is long enough.
Fix: in 5.x we'll issue a warning in such a case.
In 6.0 we should get rid of NAME_CONST().

The bug was that SQL_BIG_SELECTS was not properly set
in COM_CHANGE_USER.

Problem: storing "SELECT ... INTO @var ..." results in variables we used val_xxx()
methods which returned results of the current row.
So, in some cases (e.g. SELECT DISTINCT, GROUP BY or HAVING) we got data
from the first row of a new group (where we evaluate a clause) instead of
data from the last row of the previous group.
Fix: use val_xxx_result() counterparts to get proper results.

The problem is that a unfiltered user query was being passed as
the format string parameter of sql_print_warning which later
performs printf-like formatting, leading to crashes if the user
query contains formatting instructions (ie: %s). Also, it was
using THD::query as the source of the user query, but this
variable is not meaningful in some situations -- in a delayed
insert, it points to the table name.
The solution is to pass the user query as a parameter for the
format string and use the function parameter query_arg as the
source of the user query.

The function thd_security_context allocates memory on an unprotected MEM_ROOT if the
message length becomes longer than requested and the initial buffer memory needs to
be reallocated.
This patch fixes the design error by copying parts of the reallocated buffer
to the destination buffer. This works because the destination buffer isn't
owned by the String object and thus isn't freed when a new buffer is allocated.
Any new memory allocated by the String object is reclaimed when the object
is destroyed at the end of the function call.
Innodb monitor could cause a server crash because of invalid access to a
shared variable in a concurrent environment.
This patch adds a guard to protect against crashes but not against
inconsistent values because of performance reasons.

When statement-based replication is used, and the
transaction isolation level is READ-COMMITTED or stricter,
InnoDB will print an error because statement-based
replication might lead to inconsistency between master
and slave databases. However, when the binary log is not
engaged, this is not an issue and an error should
not be printed.
This patch makes thd_binlog_format() return BINLOG_FORMAT_
UNSPEC when the binary log is not engaged for the given
thread.

The failure was caused by executing a CREATE-SELECT statement that creates a
table in another database than the current one. In row-based logging, the
CREATE statement was written to the binary log without the database, hence
creating the table in the wrong database, causing the following inserts to
fail since the table didn't exist in the given database.
Fixed the bug by adding a parameter to store_create_info() that will make
the function print the database name before the table name and used that
in the calls that write the CREATE statement to the binary log. The database
name is only printed if it is different than the currently selected database.
The output of SHOW CREATE TABLE has not changed and is still printed without
the database name.

The assertion indicates that some data was left in the transaction
cache when the server was shut down, which means that a previous
statement did not commit or rollback correctly.
What happened was that a bug in the rollback of a transactional
table caused the transaction cache to be emptied, but not reset.
The error can be triggered by having a failing UPDATE or INSERT,
on a transactional table, causing an implicit rollback.
Fixed by always flushing the pending event to reset the state
properly.

The problem of this bug is that we need to get the list of tables
to be updated for a multi-table update statement, which requires to
open all the tables referenced by the statement and resolve all
the fields involved in update in order to figure out the list of
tables for update. However if there are replicate filter rules,
some tables might not exist on slave and result in a failure
before we could examine the filter rules.
I think the whole problem can not be solved on slave alone,
the master must record and send the information of tables
involved for update to slave, so that the slave do not need to
open all the tables referenced by the multi-table update statement to
figure out which tables are involved for update.
So a status variable is added to Query_log event to store the
value of table map for update on master. And on slave, it will
try to get the value of this variable and use it to examine
filter rules without opening any tables on slave, if this values
is not available, the old approach is used and thus the bug will
still occur for when replicating from old masters.

The crash was caused by freeing the internal parser stack during the parser
execution.
This occured only for complex stored procedures, after reallocating the parser
stack using my_yyoverflow(), with the following C call stack:
- MYSQLparse()
- any rule calling sp_head::restore_lex()
- lex_end()
- x_free(lex->yacc_yyss), xfree(lex->yacc_yyvs)
The root cause is the implementation of stored procedures, which breaks the
assumption from 4.1 that there is only one LEX structure per parser call.
The solution is to separate the LEX structure into:
- attributes that represent a statement (the current LEX structure),
- attributes that relate to the syntax parser itself (Yacc_state),
so that parsing multiple statements in stored programs can create multiple
LEX structures while not changing the unique Yacc_state.
Now, Yacc_state and the existing Lex_input_stream are aggregated into
Parser_state, a structure that represent the complete state of the (Lexical +
Syntax) parser.

The crash appeared to be a result of allocating an instance of Discrete_interval
automatically that that was referred in out-of-declaration scope.
Fixed with correcting backing up and restoring scheme of
auto_inc_intervals_forced, introduced by bug#33029, by means of shallow copying;
added simulation code that forces executing those fixes of the former bug that
targeted at master-and-slave having incompatible bug#33029-prone versions.

It is forbidden to use the SELECT INTO construction inside UNION statements
unless on the last SELECT of the union. The parser records whether it
has seen INTO or not when parsing a UNION statement. But if the INTO was
legally used in an outer query, an error is thrown if UNION is seen in a
subquery. Fixed in 5.0 by remembering the nesting level of INTO tokens and
mitigate the error unless it collides with the UNION.

prepared statement prepare and execute. If some table used in the statement
has changed, the prepared statement is re-prepared before execution.

If a binlog file is manually replaced with a namesake directory the internal purging did
not handle the error of deleting the file so that eventually
a post-execution guards fires an assert.

For all 5.0 and up to 5.1.12 exclusive, when a stored routine or
trigger caused an INSERT into an AUTO_INCREMENT column, the
generated AUTO_INCREMENT value should not be written into the
binary log, which means if a statement does not generate
AUTO_INCREMENT value itself, there will be no Intvar event (SET
INSERT_ID) associated with it even if one of the stored routine
or trigger caused generation of such a value. And meanwhile, when
executing a stored routine or trigger, it would ignore the
INSERT_ID value even if there is a INSERT_ID value available set
by a SET INSERT_ID statement.
Starting from MySQL 5.1.12, the generated AUTO_INCREMENT value is
written into the binary log, and the value will be used if
available when executing the stored routine or trigger.
Prior fix of this bug in MySQL 5.0 and prior MySQL 5.1.12
(referenced as the buggy versions in the text below), when a
statement that generates AUTO_INCREMENT value by the top
statement was executed in the body of a SP, all statements in the
SP after this statement would be treated as if they had generated
AUTO_INCREMENT by the top statement. When a statement that did
not generate AUTO_INCREMENT value by the top statement but by a
function/trigger called by it, an erroneous Intvar event would be
associated with the statement, this erroneous INSERT_ID value
wouldn't cause problem when replicating between masters and
slaves of 5.0.x or prior 5.1.12, because the erroneous INSERT_ID
value was not used when executing functions/triggers. But when
replicating from buggy versions to 5.1.12 or newer, which will
use the INSERT_ID value in functions/triggers, the erroneous
value will be used, which would cause duplicate entry error and
cause the slave to stop.
The patch for 5.1 fixed it to ignore the SET INSERT_ID value when
executing functions/triggers if it is replicating from a master
of buggy versions, another patch for 5.0 fixed it not to generate
the erroneous Intvar event.

The DBUG code emits the current value of the proc_info member of THD,
which may be set to NULL. It was wrong to dereference that value
with the format string %s without verifying that it was valid.
Now, insert an inline test that substitutes the string "(null)" for
NULL pointers.

redundant disc syncs, an optimization of the two-phase commit

It is forbidden to use the SELECT INTO construction inside UNION statements
unless on the last SELECT of the union. The parser records whether it
has seen INTO or not when parsing a UNION statement. But if the INTO was
legally used in an outer query, an error is thrown if UNION is seen in a
subquery. Fixed in 5.0 by remembering the nesting level of INTO tokens and
mitigate the error unless it collides with the UNION.

- ha_autocommit_or_rollback()
- net_end_statement()
- query_cache_end_of_result().
In future it may also include:

Rename now unused members of NET: no_send_ok, no_send_error, report_error.
These were server-specific variables related to the client/server
protocol. They have been made obsolete by this patch.
Previously the same members of NET were used to store the error message
both on the client and on the server.
The error message was stored in net.last_error (client: mysql->net.last_error,
server: thd->net.last_error).

not inheritance-safe. Even if a stored procedure continue handler is
found, the current statement is aborted, not succeeded.
Instead introduce a Protocol API to send the required response,

to add more asserts that there is no error, there is an assert inside

The story of COM_SHUTDOWN is interesting. Long story short, the server
would become on its death's door, and only no_send_ok/no_send_error assigned
by send_ok()/net_send_error() would hide its babbling from the client.
First of all, COM_QUIT does not require a response. So, the comment saying
"Let's send a response to possible COM_QUIT" is not only groundless
(even mysqladmin shutdown/mysql_shutdown() doesn't send COM_QUIT after
COM_SHUTDOWN), it's plainly incorrect.
Secondly, besides this additional 'OK' packet to respond to a hypothetical
COM_QUIT, there was the following code in dispatch_command():

This worked out really funny for the thread through which COM_SHUTDOWN
was delivered: we would get COM_SHUTDOWN, say okay, say okay again,
kill everybody, get the kill signal ourselves, and then attempt to say
"Server shutdown in progress" to the client that is very likely long gone.
This all became visible when asserts were added that the Diagnostics_area
is not assigned twice.
Move query_cache_end_of_result() to the end of dispatch_command(), since
net_send_eof() has been moved there. This is safe, query_cache_end_of_result()
is a no-op if there is no started query in the cache.
Consistently use select_send interface to call abort() or send_eof()
depending on the operation result.
Remove thd->fatal_error() from reset_master(), it was a no-op.
in hacks with no_send_error woudl save us
from complete breakage of the client/server protocol.

The problem is that DROP TABLE and other DDL statements failed to
automatically close handlers associated with tables that were marked
for reopen (FLUSH TABLES).
The current implementation fails to properly discard handlers of
dropped tables (that were marked for reopen) because it searches
on the open handler tables list and using the current alias of the
table being dropped. The problem is that it must not use the open
handler tables list to search because the table might have been
closed (marked for reopen) by a flush tables command and also it
must not use the current table alias at all since multiple different
aliases may be associated with a single table. This is specially
visible when a user has two open handlers (using alias) of a same
table and a flush tables command is issued before the table is
dropped (see test case). Scanning the handler table list is also
useless for dropping handlers associated with temporary tables,
because temporary tables are not kept in the THD::handler_tables
list.
The solution is to simple scan the handlers hash table searching
for, and deleting all handlers with matching table names if the
reopen flag is not passed to the flush function, indicating that
the handlers should be deleted. All matching handlers are deleted
even if the associated the table is not open.

On Mac OS X, KILL statement issued to kill the current
connection would return a different error code and message than on
other platforms ('MySQL server has gone away' instead of 'Shutdown
in progress').
The reason for this difference was that on Mac OS X we have macro
SIGNAL_WITH_VIO_CLOSE defined. This macro forces KILL
implementation to close the communication socket of the thread
that is being killed. SIGNAL_WITH_VIO_CLOSE macro is defined on
platforms where just sending a signal is not a reliable mechanism
to interrupt the thread from sleeping on a blocking system call.
In a nutshell, closing the socket is a hack to work around an
operating system bug and awake the blocked thread no matter what.
However, if the thread that is being killed is the same
thread that issued KILL statement, closing the socket leads to a
prematurely lost connection. At the same time it is not necessary
to close the socket in this case, since the thread in question
is not inside a blocking system call.
The fix, therefore, is to not close the socket if the thread that
is being killed is the same that issued KILL statement, even with
defined SIGNAL_WITH_VIO_CLOSE.

Originally it was the same code to handle init-connect and init-slave
mysqld options. Then init-connect implementation forked off,
but the one who copy-pasted the code didn't change it to not
use a replication-specific variable.
There actually were several problems here:
- WRITE-lock is required to load events from the mysql.event table,
but in the read-only mode an ordinary user can not acquire it;
- Security_context::master_access attribute was not properly
initialized in Security_context::init(), which led to differences
in behavior with and without debug configure options.
- if the server failed to load events from mysql.event, it forgot to
close the mysql.event table, that led to the coredump, described
in the bug report.
The patch is to fix all these problems:
- Use the super-user to acquire WRITE-lock on the mysql.even table;
- The WRITE-lock is acquired by the event scheduler in two cases:
- on initial loading of events from the database;
- when an event has been executed, so its attributes should
be updated.
Other cases when WRITE-lock is needed for the mysql.event table
happen under the user account. So, nothing should be changed there
for the read-only mode. The user is able to create/update/drop
an event only if he is a super-user.
- Initialize Security_context::master_access;
- Close the mysql.event table in case something went wrong.

There were two problems discovered by this bug:
1. Default (current) database is not fixed at the creation time.
That leads to wrong output of DATABASE() function.
2. Database attributes (@@collation_database) are not fixed at
the creation time. That leads to wrong resultset.
Binlog breakage and Query Cache wrong output happened because of
the first problem.
The fix is to remember the current database at the PREPARE-time and
set it each time at EXECUTE.
