lock_table(): Remove the constant parameter flags=0.

lock_table_resurrect(): Merge lock_table_ix_resurrect() and
lock_table_x_resurrect().

lock_rec_lock(): Only acquire LockMutexGuard if lock_table_has()
does not hold.
