This bug occurs when some trigger for table used by DML statement is created
or changed while statement was waiting in lock_tables(). In this situation
prelocking set which we have calculated becames invalid which can easily lead
to errors and even in some cases to crashes.
With proposed patch we no longer silently reopen tables in lock_tables(),
instead caller of lock_tables() becomes responsible for reopening tables and
recalculation of prelocking set.

Unfortunately these tests rely on the order in which tables are locked
by statement so they are non-determenistic and therefore should be disabled.

execution.
lock_tables():
Instead of allowing mysql_lock_tables() to silently reopen altered or dropped
tables let us notify caller and rely on that it will do reopen itself.
This solves the problem when trigger suddenly appears or changed during
mysq_lock_tables().
close_tables_for_reopen():

The idea of the patch
is that every cursor gets its own lock id for table level locking.
Thus cursors are protected from updates performed within the same
connection. Additionally a list of transient (must be closed at
commit) cursors is maintained and all transient cursors are closed
when necessary. Lastly, this patch adds support for deadlock
timeouts to TLL locking when using cursors.

cursors, lock owner can not be identified by a thread id any more,
as we must protect cursors from updates issued within the same
connection (thread). So, each cursor has its own lock identifier to
use with table level locking.

when one connection had done FLUSH TABLES WITH READ LOCK, some updates, and then COMMIT,
it was accepted but my_error() was called and so, while client got no error, error was logged in binlog.
We now don't call my_error() in this case; we assume the connection know what it does.
This problem was specific to 4.0.21. The change is needed to make replication work with existing versions of innobackup.

Bug #4810 "deadlock with KILL when the victim was in a wait state"
(I included mutex unlock into exit_cond() for future safety)
and BUG#4827 "KILL while START SLAVE may lead to replication slave crash"

we did exit_cond() before unlock(LOCK_open), which led to deadlocks with THD::awake(). Fixing this.

mutex unlock is now included in exit_cond()

we did exit_cond() before unlock(LOCK_rpl_status), which led to deadlocks with THD::awake(). Fixing this.
sql/slave.cc:
we did exit_cond() before unlock(cond_lock), which led to deadlocks with THD::awake(). Fixing this.
Fixing also that if killed while waiting for slave thread to start, we don't release the mutex
(that caused a double release of the mutex => crash).
