923fe817e0 Enable support of access to tables from triggers. Thus fix bug #8406 "Triggers
crash if referencing a table" and several other related bugs.
Fix for bug #11834 "Re-execution of prepared statement with dropped function
crashes server." which was spotted during work on previous bugs.

Also couple of nice cleanups:
- Replaced two separate hashes for stored routines used by statement with one.
- Now instead of doing one pass through all routines used in statement for
caching them and then doing another pass for adding their tables to table
list, we do only one pass during which do both things.

18d6dd56ef Fixed BUG#7088: Stored procedures: labels won't work if character set is utf8.
We used 'IDENT' for labels as a temporary fix for the parser conflicts
introduced if the proper rule 'ident' was used. Now a specially tailored
'label_ident' rule is used for labels instead.

481bb717d7 Disabled the GOTO feature. (It's non-standard and undocumented.)
We want to have the defacto standard syntax for labels ("L:" instead of "label L;"),
and fix some known bugs, before we enable this again.
The code is left intact (#ifdef'ed SP_GOTO) and the test cases are kept in
sp-goto.test, for the future...

2235ea9593 Fix bug #9728 decreased functionality in "on duplicate key update"
Remove changes made by bug fix #8147. They strips list of insert_table_list to
only insert table, which results in error reported in bug #9728.
Added flag to Item to resolve ambigous fields reported in bug #8147.

5394a1a4bd Fix for bug #11158 "Can't perform multi-delete in stored procedure".
In order to make multi-delete SP friendly we need to have all table
locks for the elements of main statement table list properly set
at the end of parsing.

Also performed small cleanup: We don't need relink_tables_for_multidelete()
any longer since the only case now when TABLE_LIST::correspondent_table
is non-zero are tables in auxilary table list of multi-delete and these
tables are handled specially in mysql_multi_delete_prepare().

270b695f59 Fix for bug #10015 "Crash in InnoDB if stored routines are used".
We should not allow explicit or implicit transaction commits inside
of stored functions or triggers (so in autocommit mode we should not
do commits after execution of sub-statement).
Also since we don't support nested statement transactions in 5.0,
we shouldn't commit or rollback stmt transactions while we are inside
stored functions or triggers. This should be fixed in later (>=5.1)
releases.

type of ST_SELECT_LEX::select_limit,offset_limit from ha_rows to Item*,
so that it can point to Item_param.

5afef58364 Fixed BUG#10969: Stored procedures: crash if default() function
Return an error if default() is used on a local variable.
This is actaully a side-effect of BUG#5967: Stored procedure declared
variable used instead of column (to be fixed later), so this is really a
  workaround until that is fixed.

0ef3267251 Fix for trigger.test failure in --debug mode.
We can't have Item_trigger_field as aggregated object inside of
sp_instr_set_trigger_field class since in this case its destructor
will be called twice. So instead let us create this Item separately
and store pointer to it in instruction object.
