02e5ad9881 Bug#43758 Query cache can lock up threads in 'freeing items' state
Early patch submitted for discussion.

It is possible for more than one thread to enter the condition
in query_cache_insert(), but the condition predicate is to
signal one thread each time the cache status changes between
the following states: {NO_FLUSH_IN_PROGRESS,FLUSH_IN_PROGRESS,
TABLE_FLUSH_IN_PROGRESS}

Consider three threads THD1, THD2, THD3

   THD2: select ... => Got a writer in ::store_query
   THD3: select ... => Got a writer in ::store_query
   THD1: flush tables => qc status= FLUSH_IN_PROGRESS;
                      new writers are blocked.
   THD2: select ... => Still got a writer and enters cond in
                       query_cache_insert
   THD3: select ... => Still got a writer and enters cond in
                       query_cache_insert
   THD1: flush tables => finished and signal status change.
   THD2: select ... => Wakes up and completes the insert.
   THD3: select ... => Happily waiting for better times. Why hurry?

This patch is a refactoring of this lock system. It introduces four new methods:
   Query_cache::try_lock()
   Query_cache::lock()
   Query_cache::lock_and_suspend()
   Query_cache::unlock()

This change also deprecates wait_while_table_flush_is_in_progress(). All threads are
queued and put on a conditional wait. On each unlock the queue is signalled. This resolve
the issues with left over threads. To assure that no threads are spending unnecessary
time waiting a signal broadcast is issued every time a lock is taken before a full
cache flush.

5c42a6cf54 Bug#37416 When SQL_NO_CACHE is used, MySQL still lookup into the query cache
The query cache module did not check for the SQL_NO_CACHE keyword before
attempting to query the hash lookup table. This had a small performance impact.

By introducing a check on the query string before obtaining the hash mutex
we can gain some performance if the SQL_NO_CACHE directive is used often.

664bb23a30 Bug#41098: Query Cache returns wrong result with concurrent insert
The problem is that select queries executed concurrently with
a concurrent insert on a MyISAM table could be cached if the
select started after the query cache invalidation but before
the unlock of tables performed by the concurrent insert. This
race could happen because the concurrent insert was failing
to prevent cache of select queries happening at the same time.

The solution is to add a 'uncacheable' status flag to signal
that a concurrent insert is being performed on the table and
that queries executing at the same time shouldn't cache the
results.

ecfdc3560c Bug#40264: Aborted cached query causes query to hang indefinitely on next cache hit
The problem is that the query cache was storing partial results
if the statement failed when sending the results to the client.
This could cause clients to hang when trying to read the results
from the cache as they would, for example, wait indefinitely for
a eof packet that wasn't saved.

The solution is to always discard the caching of a query that
failed to send its results to the associated client.

3d6cea324a Bug#36326: nested transaction and select
The problem is that the query cache stores packets containing
the server status of the time when the cached statement was run.
This might lead to a wrong transaction status in the client side
if a statement is cached during a transaction and is later served
outside a transaction context (and vice-versa).

The solution is to take into account the transaction status when
storing in and serving from the query cache.

3bad2119f9 Bug#36086: SELECT * from views don't check column grants
This patch also fixes bugs 36963 and 35600.
                      
- In many places a view was confused with an anonymous derived
  table, i.e. access checking was skipped. Fixed by introducing a
  predicate to tell the difference between named and anonymous
  derived tables.
                      
- When inserting fields for "SELECT * ", there was no 
  distinction between base tables and views, where one should be
  made. View privileges are checked elsewhere.

This bugs clarifies a warning message issued when the query cache data
size becomes smaller than the minium allowed size.

2100ec9ec9 Bug#33362: Query cache invalidation (truncate) may hang
if cached query uses many tables

The problem was that query cache would not properly cache
queries which used 256 or more tables but yet would leave
behind query cache blocks pointing to freed (destroyed)
data. Later when invalidating (due to a truncate) query cache
would attempt to grab a lock which resided in the freed data,
leading to hangs or undefined behavior.

This was happening due to a improper return value from the
function responsible for registering the tables used in the
query (so the cache can be invalidated later if one of the
tables is modified). The function expected a return value of
type boolean (char, 8 bits) indicating success (1) or failure
(0) but the number of tables registered (unsigned int, 32 bits)
was being returned instead. This caused the function to return
failure for cases where it had actually succeed because when
a type (unsigned int) is converted to a narrower type (char),
the excess bits on the left are discarded. Thus if the 8
rightmost bits are zero, the return value will be 0 (failure).

The solution is to simply return true (1) only if the number of
registered table is greater than zero and false (0) otherwise.

c184cd6320 Bug#25132 disabled query cache: Qcache_free_blocks = 1
The initial value of free memory blocks in 0. When the query cache is enabled 
a new memory block gets allocated and is assigned number 1. The free memory
block is later split each time query cache memory is allocated for new blocks.
This means that the free memory block counter won't be reduced to zero when
the number of allocated blocks are zero, but rather one. To avoid confusion
this patch changes this behavior so that the free memory block counter is
reset to zero when the query cache is disabled.
Note that when the query cache is enabled and resized the free memory block
counter was still calculated correctly.

39509d64c3 A fix and a test case for Bug#34166 Server crash in SHOW OPEN TABLES and
pre-locking.

The crash was caused by an implicit assumption in check_table_access() that
table_list parameter is always a part of lex->query_tables.

When iterating over the passed list of tables, check_table_access() used
to stop only when lex->query_tables_last_not_own was reached. 
In case of pre-locking, lex->query_tables_last_own is not NULL and points
to some element of lex->query_tables. When the parameter
of check_table_access() was not part of lex->query_tables, loop invariant
could never be violated and a crash would happen when the current table
pointer would point beyond the end of the provided list.

The fix is to change the signature of check_table_access() to also accept
a numeric limit of loop iterations, similarly to check_grant(), and 
supply this limit in all places when we want to check access of tables
that are outside lex->query_tables, or just want to check access to one table.

a3561354a2 Bug#30887 Server crashes on SET GLOBAL query_cache_size=0
Reseting the query cache by issuing a SET GLOBAL query_cache_size=0 caused the server
to crash if a the server concurrently was saving a new result set to the query cache. The
reason for this was that the invalidation wasn't waiting on the result writers to
release the block level locks on the query cache.

2a0d2fef51 Bug#12713 "Error in a stored function called from a SELECT doesn't
cause ROLLBACK of statement", part 1. Review fixes.

Do not send OK/EOF packets to the client until we reached the end of 
the current statement.
This is a consolidation, to keep the functionality that is shared by all 
SQL statements in one place in the server.
Currently this functionality includes:
- close_thread_tables()
- log_slow_statement().

After this patch and the subsequent patch for Bug#12713, it shall also include:
- ha_autocommit_or_rollback()
- net_end_statement()
- query_cache_end_of_result().

In future it may also include:
- mysql_reset_thd_for_next_command().


include/mysql_com.h:
  Rename now unused members of NET: no_send_ok, no_send_error, report_error.
  These were server-specific variables related to the client/server
  protocol. They have been made obsolete by this patch.
  
  Previously the same members of NET were used to store the error message
  both on the client and on the server. 
  The error message was stored in net.last_error (client: mysql->net.last_error,
  server: thd->net.last_error).
  The error code was stored in net.last_errno (client: mysql->net.last_errno,
  server: thd->net.last_errno).
  The server error code and message are now stored elsewhere 
  (in the Diagnostics_area), thus NET members are no longer used by the
  server.
  Rename last_error to client_last_error, last_errno to client_last_errno
  to avoid potential bugs introduced by merges.
include/mysql_h.ic:
  Update the ABI file to reflect a rename. 
  Renames do not break the binary compatibility.
libmysql/libmysql.c:
  Rename last_error to client_last_error, last_errno to client_last_errno.
  This is necessary to ensure no unnoticed bugs introduced by merged
  changesets.

mysql-test/r/events.result:
  Update to reflect the change in mysql_rm_db(). Now we drop stored
  routines and events for a given database name only if there
  is a directory for this database name. ha_drop_database() and
  query_cache_invalidate() are called likewise. 
  Previously we would attempt to drop routines/events even if database
  directory was not found (it worked, since routines and events are stored
  in tables). This fixes Bug 29958 "Weird message on DROP DATABASE if mysql.proc
  does not exist".
  The change was done because the previous code used to call send_ok()
  twice, which led to an assertion failure when asserts against it were
  added by this patch.
mysql-test/r/grant.result:
  Fix the patch for Bug 16470, now FLUSH PRIVILEGES produces an error 
  if mysql.procs_priv is missing.
  This fixes the assert that send_ok() must not called after send_error()
  (the original patch for Bug 16470 was prone to this).
mysql-test/suite/rpl/r/rpl_row_tabledefs_2myisam.result:
  Produce a more detailed error message.
mysql-test/suite/rpl/r/rpl_row_tabledefs_3innodb.result:
  Produce a more detailed error message.
mysql-test/t/grant.test:
  Update the test, now FLUSH PRIVILEGES returns an error if mysql.procs_priv

  Use getters to access statement status information.
  
  Add a comment why run_query() is broken. Reset the diagnostics area
  in the end of run_query() to fulfill the invariant that the diagnostics_area
  is never assigned twice per statement (see the comment in the code
  when this can happen). We still do not clear thd->is_fatal_error and
  thd->is_slave_error, which may lead to bugs, I consider the whole affair
  as something to be dealt with separately.
sql/ha_partition.cc:
  fatal_error() doesn't set an error by itself. Perhaps we should
  remove this method altogether and instead add a flag to my_error 
  to set thd->is_fatal_error property.
  
  Meanwhile, this change is a part of inspection made to the entire source
  code with the goal to ensure that fatal_error()
  is always accompanied by my_error().
sql/item_func.cc:
  There is no net.last_error anymore. Remove the obsolete assignment.
sql/log_event.cc:
  Use getters to access statement error status information.
sql/log_event_old.cc:
  Use getters to access statement error status information.
sql/mysqld.cc:
  Previously, if a continue handler for an error was found, my_message_sql() 
  would not set an error in THD. Since the current statement
  must be aborted in any case, find_handler() had a hack to assign 
  thd->net.report_error to 1.
  
  Remove this hack. Set an error in my_message_sql() even if the continue
  handler is found. The error will be cleared anyway when the handler
  is executed. This is one action among many in this patch to ensure the 
  invariant that whenever thd->is_error() is TRUE, we have a message in 
  thd->main_da.message().
sql/net_serv.cc:
  Use a full-blown my_error() in net_serv.cc to report an error,
  instead of just setting net->last_errno. This ensures the invariant that
  whenever thd->is_error() returns TRUE, we have a message in 
  thd->main_da.message().
  
  Remove initialization of removed NET members.
sql/opt_range.cc:
  Use my_error() instead of just raising thd->net.report_error. 
  This ensures the invariant that whenever thd->is_error() returns TRUE, 
  there is a message in thd->main_da.message().
sql/opt_sum.cc:
  Move invocation of fatal_error() right next to the place where
  we set the error message. That makes it easier to track that whenever
  fatal_error() is called, there is a message in THD.
sql/protocol.cc:
  Rename send_ok() and send_eof() to net_send_ok() and net_send_eof() 
  respectively. These functions write directly to the network and are not 
  for use anywhere outside the client/server protocol code. 
  
  Remove the code that was responsible for cases when either there is 
  no error code, or no error message, or both.
  Instead the calling code ensures that they are always present. Asserts
  are added to enforce the invariant.
  
  Instead of a direct access to thd->server_status and thd->total_warn_count
  use function parameters, since these from now on don't always come directly
  from THD.
  
  Introduce net_end_statement(), the single-entry-point replacement API for 
  send_ok(), send_eof() and net_send_error().
  
  Implement Protocol::end_partial_result_set to use in select_send::abort()

  The story of COM_SHUTDOWN is interesting. Long story short, the server 
  would become on its death's door, and only no_send_ok/no_send_error assigned
  by send_ok()/net_send_error() would hide its babbling from the client.
  
  First of all, COM_QUIT does not require a response. So, the comment saying
  "Let's send a response to possible COM_QUIT" is not only groundless 
  (even mysqladmin shutdown/mysql_shutdown() doesn't send COM_QUIT after 
  COM_SHUTDOWN), it's plainly incorrect.
  
  Secondly, besides this additional 'OK' packet to respond to a hypothetical
  COM_QUIT, there was the following code in dispatch_command():
  
  if (thd->killed)
    thd->send_kill_message();
  if (thd->is_error()
    net_send_error(thd);
  
  This worked out really funny for the thread through which COM_SHUTDOWN
  was delivered: we would get COM_SHUTDOWN, say okay, say okay again, 
  kill everybody, get the kill signal ourselves, and then attempt to say 
  "Server shutdown in progress" to the client that is very likely long gone.
  
  This all became visible when asserts were added that the Diagnostics_area
  is not assigned twice.
  
  Move query_cache_end_of_result() to the end of dispatch_command(), since
  net_send_eof() has been moved there. This is safe, query_cache_end_of_result()
  is a no-op if there is no started query in the cache.
  
  Consistently use select_send interface to call abort() or send_eof()
  depending on the operation result.
  
  Remove thd->fatal_error() from reset_master(), it was a no-op. 
  in hacks with no_send_error woudl save us
  from complete breakage of the client/server protocol.
  
  Consistently use select_send::abort() whenever there is an error, 
  and select_send::send_eof() in case of success.
  The issue became visible due to added asserts.

1f8126190b Bug#30710 query_cache.test fails on embedded - per-column privs test
The embedded version of the server doesn't use column level grants, and 
the compile directive NO_EMBEDDED_ACCESS_CHECKS should be checked instead of
the redundant HAVE_QUERY_CACHE (which is always the case) to determine if 
column level grants should be compiled or not.

319c6ef199 Bug #30768 query cache patch for bug #21074 crashes on windows
"pthread_mutex_trylock" isn't implemented correctly for the Windows platform.

This temporary patch reverts the patch for bug#21074 for Windows until
pthread_mutex_trylock is properly implemented.

  Added code branch for the Windows platform to avoid a faulty pthread_mutex_trylock
  implementation.

733bd4fe81 Bug #21074 Large query_cache freezes mysql server sporadically under heavy load
Invaldating a subset of a sufficiently large query cache can take a long time.
During this time the server is efficiently frozen and no other operation can
be executed. This patch addresses this problem by setting a time limit on
how long time a dictionary access request can take before giving up on the 
attempt. This patch does not work for query cache invalidations issued by
DROP, ALTER or RENAME TABLE operations.


sql/sql_cache.cc:
  Changed mutex locking to a timed spinn lock. If access to query cache dictionary
  takes "a long time" (currently more than 0.1 seconds) the system fall backs on
  ordinary statement execution.

891b1659fc Fixed bug #30201.
Killing a SELECT query with KILL QUERY or KILL CONNECTION
causes a server crash if the query cache is enabled.

Normal evaluation of a query may be interrupted by the
KILL QUERY/CONNECTION statement, in this case the mysql_execute_command
function returns TRUE, and the thd->killed flag has true value.
In this case the result of the query may
be cached incompletely (omitting call to query_cache_insert inside
the net_real_write function), and next call to query_cache_end_of_result
may lead to server crash.
Thus, the query_cache_end_of_result function has been modified to abort
query cache in the case of killed thread.

  cache in the case of query execution failure. Also this function has been
  modified to remove incomplete query block.

3a5a0ea392 Bug#30269 Query cache eats memory
Although the query cache doesn't support retrieval of statements containing
column level access control, it was still possible to cache such statements
thus wasting memory.
  
This patch extends the access control check on the target tables to avoid
caching a statement with column level restrictions. 

Views are excepted and can be cached but only retrieved by super user account.
