WL#2335 (wait if binlog or binlog index file hits disk full or quota exceeded),
fix for BUG#7236 ("--expire_logs_days does not apply if all statements
happen in transactions"), and a behaviour change: abort if mysqld can't start
binlog at startup (if running with --log-bin of course).

2 small fixes for binlog:
1) We create binlog and binlog index file with flag MY_WAIT_IF_FULL, so that they have the same
behaviour as MyISAM tables when disk is full or quota exceeded (wait, try to finish write
every minute, warn in error log every 10 minutes). That's WL#2335.
2) Honour expire-log-days when we write a transaction to binlog (we honoured it already when writing
an autocommit statement). This fixes BUG#7236.
sql/mysqld.cc:
If we fail to create binlog or binlog index file or write to them at mysqld's startup, mysqld
will exit. The former behaviour was to print error and continue, which led to stupid things like
if you mispell a directory name in --log-bin, you don't immediately notice your mistake and
you corrupt your recovery/replication. New behaviour is consistent with if you mispelt innodb_data_file_path.

Now thd->mem_root is a pointer to thd->main_mem_root and THR_MALLOC is a pointer to thd->mem_root.
This gives us the following benefits:
- Allow us to easily detect if arena has already been swapped before (this fixes a bug in setup_conds() where arena was swaped twice in some cases)
- Faster swaps of arenas (as we don't have to copy the whole MEM_ROOT)
- We don't anymore have to call my_pthread_setspecific_ptr(THR_MALLOC,...) to change where memory is alloced. Now it's enough to set thd->mem_root

Adding a compilation define so that on recent Darwin mysqld does not react to SIGHUP and SIGQUIT:
this fixes a rpl000001 problem on our Powermac G5 machine (popping after an upgrade from 10.3.4 to
10.3.5) and is expected to fix
BUG#2030 "relay bin log on slave resets multiple times a second"
(i.e. under some Mac OS X Panther versions, mysqld receives many SIGHUP and SIGQUIT).
So this fix is more a problem-hider than a real understanding of why mysqld receives so many signals.
Note that we saw other problems on this OS where mysqld reacts to Ctrl-Z but apparently only once,
where using SSL seems to make the problem more frequent...

I am noticing a failure of bdb.test; I believe this is not related to the
code I added yesterday and today, but I am checking. In any case I push this
changeset as it cannot by itself bring more mess that I *may* (or not)
already have brought with previous pushes.

a limitation of yesterday's implementation:
if there was an unfinished transaction (COMMIT not typed), and some MyISAM tables were
then updated, and then mysqld crashes, then at restart the server would use the too old
binlog offset known by InnoDB to cut the binlog, thus cutting the successful MyISAM
updates. We fix this by reporting the binlog offset into InnoDB even if InnoDB was not
affected at all by the update.
But the feature is still disabled until we decide if it can go into 4.1.3.

Now when mysqld starts, if InnoDB does a crash recovery, we use the binlog name
and position retrieved from InnoDB (corresponding to the last transaction
successfully committed by InnoDB) to cut any rolled back transaction from
the binary log. This is triggered by the --innodb-safe-binlog option.
Provided you configure mysqld to fsync() InnoDB at every commit (using
flush_log_at_trx_commit) and to fsync() the binlog at every write
(using --sync-binlog=1), this behaviour guarantees that a master always has
consistency between binlog and InnoDB, whenever the crash happens.
6 tests to verify that it works.
