b5a038a590 Bug #54282 Crash in MDL_context::upgrade_shared_lock_to_exclusive
This crash happened if a table was listed twice in a DROP TABLE statement,
and the statement was executed while in LOCK TABLES mode. Since the two
elements of table list were identical, they were assigned the same TABLE object. 
During processing of the first table element, the TABLE instance was destroyed
and the second table list element was left with a dangling reference.
When this reference was later accessed, the server crashed.

Listing the same table twice in DROP TABLES should give an ER_NONUNIQ_TABLE
error. However, this did not happen as the check for unique table names was 
skipped due to the lock type for table list elements being set to TL_IGNORE.
Previously TL_UNLOCK was used and the unique check was performed.

This bug was a regression introduced by a pre-requisite patch for
Bug#51263 "Deadlock between transactional SELECT and ALTER TABLE ...
REBUILD PARTITION". The regression only existed in an internal team
tree and never in any released code.

This patch reverts DROP TABLE (and DROP VIEW) to the old behavior of
using TL_UNLOCK locks. Test case added to drop.test.

db1888b53c Bug #49988 MDL deadlocks with mysql_create_db, reload_acl_and_cache
This was a deadlock between LOCK TABLES/CREATE DATABASE in one connection
and DROP DATABASE in another. It only happened if the table locked by 
LOCK TABLES was in the database to be dropped. The deadlock is similar
to the one in Bug#48940, but with LOCK TABLES instead of an active
transaction.

The order of events needed to trigger the deadlock was:
1) Connection 1 locks table db1.t1 using LOCK TABLES. It will now
have a metadata lock on the table name.
2) Connection 2 issues DROP DATABASE db1. This will wait inside
the MDL subsystem for the lock on db1.t1 to go away. While waiting, it
will hold the LOCK_mysql_create_db mutex.
3) Connection 1 issues CREATE DATABASE (database name irrelevant).
This will hang trying to lock the same mutex. Since this is the connection
holding the metadata lock blocking Connection 2, we have a deadlock.

This deadlock would also happen for earlier trees without MDL, but 
there DROP DATABASE would wait for a table to be removed from the
table definition cache.

This patch fixes the problem by prohibiting CREATE DATABASE in LOCK TABLES
mode. In the example above, this prevents Connection 1 from hanging trying
to get the LOCK_mysql_create_db mutex. Note that other commands that use
LOCK_mysql_create_db (ALTER/DROP DATABASE) are already prohibited in 
LOCK TABLES mode.

Incompatible change: CREATE DATABASE is now disallowed in LOCK TABLES mode.

Test case added to schema.test.

Bug#25858 Some DROP TABLE under LOCK TABLES can cause deadlocks
        
When a client (connection) holds a lock on a table and attempts to
drop (obtain a exclusive lock) on a second table that is already
held by a second client and the second client then attempts to
drop the table that is held by the first client, leads to a
circular wait deadlock. This scenario is very similar to trying to
drop (or rename) a table while holding read locks and are
correctly forbidden.
        
The solution is to allow a drop table operation to continue only
if the table being dropped is write (exclusively) locked, or if
the table is temporary, or if the client is not holding any
locks. Using this scheme prevents the creation of a circular
chain in which each client is waiting for one table that the
next client in the chain is holding.
            
This is incompatible change, as can be seen by number of tests
cases that needed to be fixed, but is consistent with respect to
behavior of the different scenarios in which the circular wait
might happen.

  The problem was that the high-level function mysql_rm_db() invoked
  low-level mysql_rm_table_part2(), which reported low-level error
  (Unknown table) if SE refused to delete a table. Also when
  mysql_rm_table_part2() reported an error, it didn't add corresponding
  warning into the list (because it is used from other places where such
  behaviour is required).
  
  The fix is to
    1. Remove no_warnings_for_error usage from sql_table.cc
    2. Improve internal error handler support in THD, so that
       a stack of error handlers is allowed.
    3. Create an internal error handler (Drop_table_error_handler)
       to silence useless warnings.
    4. Use the handler in DROP DATABASE and DROP TABLE statements.

02a902edc1 Fix for Bug#42308 Several server tests do not pass MTR's --check option

The test modified here failed in stability (high parallel load) tests.

1. Take care that disconnects are finished before the test terminates.

05258bfa61 Bug#26703: DROP DATABASE fails if database contains a #mysql50# \
table with backticks

(Thanks to Lu Jingdong, though I did not take his patch directly, as
it contained a significant flaw.)

It wasn't a backtick/parsing problem.  We merely didn't anticipate
and allocate enough space to handle the optional "#mysql50#" table-
name prefix. 

Now, allocate that extra space in case we need it when we look up 
a legacy table to get its file's name.

6217dd728a Bug#33464: DROP FUNCTION caused a crash
The cause of the crash is an assertion failure that we do not emit 
an error message (grant not found) and then return "ok".  The 
assertion is valid, and we were ignoring the buggy behavior prior 
to the "Diagnostics" result-verification.

Use an error handler to mutate innocuous missing-grant errors, when 
removing routines, into warnings.

186fa6c291 Fix for bug#21216 "Simultaneous DROP TABLE and SHOW OPEN TABLES causes
server to crash".

Crash caused by assertion failure happened when one ran SHOW OPEN TABLES
while concurrently doing DROP TABLE (or RENAME TABLE, CREATE TABLE LIKE
or any other command that takes name-lock) in other connection.
For non-debug version of server problem exposed itself as wrong output
of SHOW OPEN TABLES statement (it was missing name-locked tables).
Finally in 5.1 both debug and non-debug versions simply crashed in
this situation due to NULL-pointer dereference.

This problem was caused by the fact that table placeholders which were
added to table cache in order to obtain name-lock had TABLE_SHARE::table_name
set to 0. Therefore they broke assumption that this member is non-0 for
all tables in table cache which was checked by assert in list_open_tables()
(in 5.1 this function simply relies on it).
The fix simply sets this member for such placeholders to appropriate value
making this assumption true again.

This patch also includes test for similar bug 12212 "Crash that happens
during removing of database name from cache" reappeared in 5.1 as bug 19403.

ee0f6217ea I was wrong about concurrent insert issue in rpl000016 - it still failed
once after the lock. Now changed test case and added eval support to
mysqltest to make it easier to debug
