Item_xml_str_func::fix_fields() used a local "String tmp" as a buffer
for args[1]->val_str(). "tmp" was freed at the end of fix_fields(),
while Items created during my_xpath_parse() still pointed to its fragments.
Adding a new member Item_xml_str_func::m_xpath_query and store the result
of args[1]->val_str() into it.
TODO:

If compiling a non DBUG binary with
-DDBUG_ASSERT_AS_PRINTF asserts will be
changed to printf + stack trace (of stack

- Item::name_length was not kept up to date. Now fixed
- Several bugs found and fixed (Access to null pointers,
access of freed memory, wrong arguments to printf like functions)
- Removed a lot of casts from (const char*) to (char*)

The XML parser position stack for each level is with a fixed depth.
So a bounds check was done to ensure that this depth is not exceeded.
But it was off by one (i.e. the size of the array was a valid index).
Fixed by decreasing the allowable depth by one to match the maximum
number of elements in the position stack.
MDEV-10134 Add full support for DEFAULT
- Added support for using tables with MySQL 5.7 virtual fields,
including MySQL 5.7 syntax

pthread_getspecific() calls from 290 to 177 per OLTP RO transaction.

- Changed ER(ER_..) to ER_THD_OR_DEFAULT(current_thd, ER...) in some places where current_thd is not necessary defined.

Part of this is optimization (not calling current_thd when not needed),
but part is bug fixing for error condition when current_thd is not defined
(For example on startup and end of mysqld)

Other things:
- Added DBUG_ASSERT(thd == tmp_thd) in Item_singlerow_subselect() just to be safe.

The reason for the failure was a bug in an include file on debian that causes 'struct stat'
to have different sized depending on the environment.
This patch fixes so that we always include my_global.h or my_config.h before we include any other files.
Other things:
- Removed #include <my_global.h> in some include files; Better to always do this at the top level to have as few
"always-include-this-file-first' files as possible.

MDEV-6666 Malformed result for CONCAT(utf8_column, binary_string)
Item_static_string_func::safe_charset_converter() and
Item_hex_string::safe_charset_converter() did not
handle character sets with mbminlen>1 properly, as well as
did not handle conversion from binary to multi-byte well.
Introducing Item::const_charset_converter(), to reuse it in a number
of Item_*::safe_charset_converter().
- MDEV-5689 ExtractValue(xml, 'substring(/x,/y)') crashes
- MDEV-5709 ExtractValue() with XPath variable references returns wrong result.
Description:
1. The main problem was that that nodeset_func->fix_fields() was
called in Item_func_xml_extractvalue::val_str() and
Item_func_xml_update::val_str(), which led in some cases to
execution of the XPath engine *before* having a parsed XML value.
Moved to Item_xml_str_func::fix_fields().
2. Cleanup: added a new method Item_xml_str_func::fix_fields() and moved
most of the code from Item_xml_str_func::fix_length_and_dec()
to Item_xml_str_func::fix_fields(), to follow the usual Item layout.
3. Cleanup: a parsed XML value is useless without the raw XML value
it was built from.
Previously the parsed and the raw values where stored in separate String
instances. It was hard to follow how they are synchronized.
Added a helper class XML which contains both parsed and raw values.
Makes things easier to read and modify.
4. MDEV-5709: const_item() could incorrectly return a "true"
result when XPath expression contains users/SP variable references.
Now nodeset_func->const_item() is also taken into account to
catch such cases.
5. Minor code enhancements.
MALFORMED XPATH EXP
Problem:
A malformed XPATH expression in the ExtractValue query is causing
a server crash. This malformed XPATH expression is resulted when
the position attribute in the substring function contains ".." in
the beginning.
Solution:
The original crash is happening because the "../" is being evaluated
prematurely. It tries to access XML while it hasn't been parsed yet.
The premature evaluation is happening because the val_nodeset function
is being set to constant, in which case we proceed to evaluate them in
JOIN:prepare stage only. The solution to this is setting the val_nodeset
functions as non-constant. This forces us to evaluate the function in
the JOIN:exec stage and thus avoid any premature evaluation of the
XML strings.

a server crash. This malformed XPATH expression is resulted when
the position attribute in the substring function contains ".." in
