object into persistent arena of PS.

Implemented support for execution of statements in "prelocked" mode.
When we have statement which uses stored routines explicitly or
implicitly (via views or triggers) we have to open and lock all tables
for these routines at the same time as tables for the main statement.
In fact we have to do implicit LOCK TABLES at the begining of such
statement and implict UNLOCK TABLES at its end. We call such mode
"prelocked".
When open_tables() is called for the statement tables which are needed
for execution of routines used by it are added to its tables list
(this process also caches all routines used). Implicit use of routines
is discovered when we open view or table with trigger and apropriate
tables are added to the table list at this moment. Statement which has
such extra tables in its list (well actually any that uses functions)
is marked as requiring prelocked mode for its execution.
When lock_tables() sees such statement it will issue implicit LOCK TABLES
for this extended table list instead of doing usual locking, it will also
set THD::prelocked_mode to indicate that we are in prelocked mode.
When open_tables()/lock_tables() are called for statement of stored
routine (substatement), they notice that we are running in prelocked mode
and use one of prelocked tables from those that are not used by upper
levels of execution.
close_thread_tables() for substatement won't really close tables used
but will mark them as free for reuse instead.
Finally when close_thread_tables() is called for the main statement it
really unlocks and closes all tables used.
Everything will work even if one uses such statement under real LOCK
TABLES (we are simply not doing implicit LOCK/UNLOCK in this case).

This patch contains all that my previous patch (1.1814) contained, with the addition of using cli_fetch_lengths for
handling binary data (Bar noted this on the review of 1.1814, Guilhem suggested using cli_fetch_lenghts by
making available via removal of static in method definition and declaration in mysql.h, but
Konstantin had some reservations, but he said to commit the patch using this anyway,
and I suppose this can be discussed. I abandoned 1.1814 because Monty made a couple
fixes to my code as well as formatting changes, and I thought it would just be easier
to hand-edit my changes into a fresh clone and then make a patch.
The reason for using cli_fetch_lengths is so that I can correctly get the length of
the field I am setting into the field. I was previously using 'strlen' but Bar pointed out this
won't correctly get the length of binary data and is also less effecient. Upon testing,
it was in fact verified that binary data in a blob table was being inserted correctly,
but not being retrieved correctly, all due to not having the correct value for the
field:
(*field)->store(row[x], strlen(row[x]), &my_charset_bin);
was changed to:
(*field)->store(row[x], lengths[x], &my_charset_bin);
lengths being a unsigned long pointer to the values of the field lengths from a
MYSQL_ROW.
Since the server doesn't have the function "mysql_fetch_lengths" available, I tried
to use "result->lengths", but this isn't set, so I finally successfully used
cli_fetch_lenghts, which does give the correct lengths, and now the binary data gets
retrieved correctly.
I've also run the code through indent-ex and am using Brian's vimrc to ensure correct formatting!
This code passes the entire test suite, without any errors or warning on both my
workstation and build.mysql.com

we store 7 bytes (1 + 2*3) in every Query_log_event.
In the future if users want binlog optimized for small size and less safe,
we could add --binlog-no-charset (and binlog-no-sql-mode etc): charset info
is something by design optional (even if for now we don't offer possibility to disable it):
it's not a binlog format change.
We try to reduce the number of get_charset() calls in the slave SQL thread to a minimum
by caching the charset read from the previous event (which will often be equal to the one of the current event).
We don't use SET ONE_SHOT for charset-aware repl (we still do for timezones, will be fixed later).
No more errors if one changes the global value of charset vars on master or slave
(as we log charset info in all Query_log_event).

sql/log_event.cc:
We now log charset info in each Query_log_event in binlog. It's 2*3 = 6 bytes:
session character_set_client, session collation_connection, session collation_server.
Now we would need only one byte per variable, but Bar said 2 is safer for the future.
When slave or mysqlbinlog reads that info, it needs to get_charset() on these numbers (so, 3 get_charset() calls),
as most of the time the 6-byte charset info will be equal to the previous event's,
we cache the previous event's charset and if equal, no need to get_charset().
As "flags2", SQL_MODE, catalog, autoinc variables, charset info is not a permanent addition:
in the future we can add options to the master to not log any of these, old 5.0 should be able
to parse these.
A little bit of cleanup on autoinc stuff in replication.
Fixing a bug in Start_log_event_v3::exec_event() where we used rli->relay_log.description_event_for_exec->binlog_version
while we should use binlog_version (if it's a 3.23 master, that's all that counts; not the fact that the relay log is
in 5.0 format).

Yes this opens a breach if one does 4.1->5.0.3, where the checks would still be needed. But these checks would need
reading relay_log.description_event_for_queue, which is currently an object used in many places by the I/O
thread and only it. So, currently we don't take mutexes for this object, and if we read the object in set_var.cc
(client thread) we need to add mutexes everywhere, but the replication code is already too broken with mutexes
now (no consistent use of mutexes); mutex usage in replication should be fixed but preferrably during/after
multimaster coding as it's going to shuffle mutexes already.

- Added a thread that fetches commit_count for open tables. This
will mean that NDB will not have to be contacted for every use of a cached query.
